Church Road Primary School provides personalised SEND provision with the SENCo and staff familiar with each pupil’s needs. Identifying and assessing need involves multiple methods (liaison with previous settings, age-related expectations, observations, concerns from parents/teachers, external agencies). The school uses Quality First Teaching, small group sessions, and targeted strategies to support learning. External services (Speech and Language Therapy, Educational Psychologist, CAMHS, Occupational Therapy, School Nursing, Physiotherapy, etc.) may be involved. The Local Authority can carry out statutory assessments if needed. Transitions (nursery → reception; end of key stages; secondary) are planned with involvement from staff and outside professionals as required. The Learning Mentor supports emotional well-being; IPM reviews and person-centred reviews (PCR) may be used for plans and progress.
