Children will initially be placed on the SEND register in school after the Registration of Initial Concern form is completed and signed by the school SENDCo and parents. They will be placed on SEN support and follow the ASSESS, PLAN, DO, REVIEW graduated approach with an SFP (School Focus Plan) that has targets/outcomes and is reviewed three times through the academic year. A person-centred approach is used and pupils are involved in reviewing SFPs and their one-page profiles. Class teachers provide Quality First Teaching, with additional targeted work and small-group/individual interventions as needed. The support may include curriculum/environment adaptations (e.g., coloured paper for Irlens Syndrome), small group or individual work, and interventions planned and reviewed every half-term. SEMH support is provided with PSHE sessions, Thrive-led sessions and a lunchtime Hub; sensory diets and a calm zone are available; specialist groups from outside agencies (e.g., Speech and Language Therapy, Occupational Therapy) may be involved. Some pupils may receive Education, Health and Care Plan (EHCP) support if required, including possible statutory assessment and referrals to external professionals. Parents may be invited to meetings to discuss progress and plan forward.
